Description
Solihull Council currently commissions a Dementia Support Service to provide support for people in Solihull who have dementia and their carers. The service includes: o A Dementia Navigator Service: The role of the Dementia Navigator Service is to provide support across the whole span of an individual's dementia journey, working directly with carers even when the person with dementia does not take up support or is unable to participate. The support is tailored to each individual according to their needs, this may include: o Advising people where they can access support following a diagnosis. o Providing access to activities which are suitable. o Assisting people to self-manage their condition. The intensity of support is provided on a tiered basis as follows: o Tier 1 - telephone and email support. o Tier 2 - more intensive face-to-face support. o Tier 3 - staying in contact with people, with access to increased support available if required.
