Szczegółowy zakres zamówienia, w tym wykaz aparatury medycznej objętej przedmiotem zamówienia, przedstawia Załącznik Nr 3 do SIWZ – Opis przedmiotu zamówienia. Pozostałe wymogi dotyczące wykonania przedmiotu zamówienia określa wzór umowy, stanowiący Załącznik Nr 9 do SWZ.
