Sectia Recuperare din Zalau, str. Simion Barnutiu nr. 65 - NLC 7001228993 (JT); 4. Cabinete ambulatoriu din Zalau, str. Pacii nr. 10 - NLC 7003604106 (JT); 5. Sectia Psihiatrie din Zalau, str. Gheorghe Doja nr. 110 – NLC 7000907776 (JT); 6. Cabinete ambulatoriu din Cehu Silvaniei, str. 1 Dec. 1918 – NLC 7003623070 (JT); Durata contractului de furnizare este de 12(doisprezece) luni de la data semnarii lui de catre ambele parti. Valoarea estimata a contractului care se estimeaza a fi incheiat este de 1.771.000,00 lei.
