Simion Barnutiu nr. 67 - NLC 3893414; 2. Sectiile TBC, din Zalau, str. Tudor Vladimirescu nr. 28 - NLC 3893419; 3. Sectia Recuperare din Zalau, str. Simion Barnutiu nr. 65 - NLC 3893418; 4. Sectia Contagioase din Zalau, str. Pacii nr. 10 - NLC 8215587; 5. Locuinta de serviciu in Zalau, str. Pietris, bloc GD 8, ap.12 - NLC 8215571; 6. Sectia Psihiatrie din Zalau, str. Gheorghe Doja nr. 110 – NLC 3867803; 7.Cabinete ambulatoriu din Cehu Silvaniei, str. 1 Dec. 1918 – NLC 7003623070. Durata contractului de furnizare este de 12(doisprezece) luni de la data semnarii lui de catre ambele parti. Valoarea estimata a contractului care se estimeaza a fi incheiat este de 1.613.445,38 lei, conform Anexei 1 a Caietului de sarcini.
