1. Przedmiotem zamówienia jest dostawa wyrobów medycznych jednorazowego użytku (CPV: 33.19.00.00-8) na potrzeby Copernicus Podmiot Leczniczy Sp. z o.o. w Gdańsku, których szczegółowy opis, asortyment i szacunkowe ilości zawiera Formularz asortymentowo-cenowy - załącznik nr 1 do SWZ (Części 1-145). 2. Przedmiot zamówienia określony w częściach nr 1-67, 69-89, 91-97, 99- 104, 107-143, 145 dotyczy sprzedaży, w częściach nr 68, 90 dotyczy dzierżawy, w częściach 98, 105, 106, 144 dotyczy komisu. 3. Wykonawca wypełniając pozycje w kolumnie „Opisać tak/nie” potwierdza spełnianie wymagań określonych przez Zamawiającego – jeżeli dotyczy. 4. Wykonawca zobowiązany jest przy pierwszej dostawie do dostarczenia wykazu sprzętu medycznego – jeżeli dotyczy. 5. Maksymalny termin dostawy zamówienia częściowego 5 dni roboczych (z uwzględnieniem zapisów w rozdziale XVIII SWZ – termin dostawy stanowi jedno z kryteriów oceny ofert).
