Description
Please note that the procurement will be using the Light Touch Regime. Please Note that the contract value is a guide only and those submitting a tender will need to calculate their own value. The provision of the service is to enable a flexible and responsive approach to those people who need short term support living in the community that may result in admission to a residential home or hospital, if need remains unmet. This adds an additional support mechanism for teams working with people in the community to help them remain safe at home, avoid hospital admission where safe to do so, and also to support people being discharged from hospital. The service is primarily for people with an urgent need for example those referred into intermediate care, urgent care response, hospital discharge or adult social care. The service will provide a comprehensive range of support, encompassing assistance with personal care, the management and administration of both prescribed and over-the-counter medications, delegated health tasks (if staff are properly trained), and practical aids for various daily activities.
