Description

NHS Humber and North Yorkshire Integrated Care Board (hereafter the Contracting Authority) is seeking to commission Support at Home (Domiciliary Care) Services - North East Lincolnshire through an Open procurement process. The four Lots to be procured are: 1. Geographical areas a. Geographical area West (plus one Extra Care Housing scheme within this area) b. Geographical area East c. Geographical area South (plus one Extra Care Housing scheme within this area) 2. Enhanced Support at Home Provision 3. Carers Breaks Provision 4. System Resilience Provision The preferred Bidders will be required to demonstrate they are able to provide a high-quality, culturally sensitive Support at Home (Domiciliary Care) services that meets CQC regulatory requirements, and the principles as defined by the Care Act 2014. Bidders will demonstrate achievement of individualised outcomes as defined by the care/support plan and will demonstrate an ability to maximise potential in relation to physical, intellectual, emotional and social capacity, to support people to remain in their homes for longer.
