Description
PRIOR INFORMATION NOTICE This is not a call for competition and is not yet a contract opportunity. The current contracts for Reablement Service (Intermediate Care - Pathway 1) in Somerset expire at the end of August 2025 and we have a commissioning plan underway to have a new contract(s) in place from 01st September 2025. Over the last year we have been working with Newton Europe to design new approaches and tools to support flow and effectiveness, as well develop a range of reporting that enables providers to both case manage and deliver good outcomes. This work has been fundamental in developing a new model for this service and we would like to share initial ideas with a view to focussing in on the proposed scope of the re-commissioning activity, the proposed approach to the geography of Somerset and the benefits and challenges of the proposals in the delivery of homecare services in the future.
