Description

This offer is for the provision of domiciliary care under the Pan-London Domiciliary Care Any Qualified Provider (AQP) contract. The Pan-London Domiciliary Care AQP contract is used by the Contracting Authorities as the primary route to commissioning home care packages, including for Discharge to Assess (D2A) and NHS Continuing Healthcare (CHC). This qualification process is being undertaken by the NHS London Purchased Healthcare (LPH) team on behalf of the following Contracting Authorities: -NHS North Central London Integrated Care Board -NHS North East London Integrated Care Board -NHS South West London Integrated Care Board
