Description
South Yorkshire Integrated Care Board (SY ICB) Rotherham place is seeking to appoint a single provider for the provision of a Social Prescribing Service. The Service will enhance the health and wellbeing of the residents of Rotherham and help decrease health inequalities, by providing a liaison and support service for people at high risk of unplanned urgent and emergency care to access non-medical sources of support in the community.
