Description
NHS South West London Integrated Care Board (ICB) (Croydon Place) is seeking to commission Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP), LTC Self-Management and Pentathlon Healthy Lifestyle Programme Services. Since 2020, the LTC Community Outreach and Expert Patients Programme has been focussing on prevention, early identification and improved management of LTCs in Croydon with the aim to raise awareness with people who do not regularly access GPs or local NHS services. With the population in Croydon with two or more long term conditions at 23% and set to increase, a collaborative approach across all long-term conditions is key to manage, support and prevent increases in the number of Long-Term Conditions and the costs associated with that to health and the individual.
