Description
This is a market engagement for Cambridgeshire and Peterborough ICB. We are looking for a Partner to help us deliver a Digital Front Door (DFD) as a single entry-point for citizens managing their care. We estimate this would be across a 5-year period (+extension option), with the value being split across this timeframe. The DFD aims to provide access to a set of products, services, and relationships with partners in the delivery of health and care which help us co-ordinate care, personalise interventions and actions and enable us to centralise some services to make them more efficient. Some of these products, services and partnerships work well in our area now. In some areas we have gaps, duplication, and lack of integration, which can make it difficult for people to interact effectively with us. They also present challenges to staff working in our care system. Cambridgeshire & Peterborough is home to circa one million people representing highly diverse communities. The ICB is aware that 112,000 people across Cambridgeshire and Peterborough live in the 20% most deprived quintile, 18.4% of our population is aged 65+, 9.1% of the population use English as a second language and we…
