Description
Suffolk County Council wish to undertake Market Engagement to provide a dedicated therapy service to aid the recovery of people following an episode of acute care. The main focus will be on the assessment and treatment of people in commissioned beds with a focus on reablement, patient centred goal setting and maximising people's independence, serving the pathway 2 discharge to assess (D2A) model.
