Keeping Well Services (Market Engagement Questionnaire)

Description

The detail of the future Keeping Well Service model in Sunderland has not yet been determined, however it may include services to provide care and support to adults aged 18 years and over who are living in their own home, specifically adults who have care and support needs arising as a result of: · Frailty · a physical disability · a sensory impairment · a mental health need, including dementia/cognitive impairment. · a learning disability including autism and challenging behaviour. · substance misuse It is not intended that the services that may be included in the Keeping Well Service are specific reablement services, but to support the achievement of the three key priority areas in our Living Well and Ageing Well Strategy for Adult Social Care in Sunderland we will expect all care and support services to be delivered with a reablement ethos.
