Description
1) This model delivers a clinical outreach programme which takes healthcare services to individuals with a view to improving access to primary care for those on the streets. With access to 4 commissioned hostel beds, the GP practices deliver a clinical programme which is responsive to changing need and circumstances. The specialist GP practices work in partnership with one another and system partners (who provide assistance such as taking patients to their health appointments), hospital Health Inclusion Teams, CLCH, CNWL, Local Authorities, and Public Health. 2) Due to the nature of the health care services the existing providers are the only realistic capable provider for the relevant authority’s specific requirements, therefore direct award process A applies. The relevant authority holds the view that these existing providers are uniquely positioned to meet the requirements of the service, and there are no viable alternatives capable of delivering the same level of quality and continuity.
