Description
The service is for GP medical provision of Pathway 2 discharge to assess (D2A) bed base for Stockport patients residing at GP medical cover to Bramhall Manor care home residents. This contract will be for 15-months from 1st January 2024 to 31st March 2025. The total value of this contract will be £101,415. This is a Provider Selection Regime (PSR) Direct Award Process C intention to award notice, this notice serves as an intention to award under the PSR.
