Description
End of Life and Palliative Care Services - Inpatient Care The service will provide a ten bedded inpatient unit for terminal care, symptom control, assessment and crisis care for individuals who have a progressive terminal condition with specialist palliative care needs. The service provided covers the population of North Lincolnshire and East Riding of Yorkshire. End of Life and Palliative Care Services - Daycare The service will provide access to specialist end of life and palliative care daycare services which will include assessment, therapy, counselling and social support. The service provided covers the population of North Lincolnshire. Community Lymphoedema Service The service will provide a community lymphoedema service which supports the specialist management of individuals living with lymphoedema, lipoedema and chronic oedema aged 18 and above. The service will be delivered in line with British Lymphology Society guidelines. The service provided covers the population of North Lincolnshire.
