Description
The Camden Adult Pathway Partnership (CAPP) is a small clinical team that provides nurse-led outreach to 16 supported accommodation sites across Camden, known collectively as the Adult Pathway. The service provides a health outreach model with a multi-disciplinary approach for single homeless people with complex needs, including ex rough sleepers.
