Description
Caerphilly County Borough Council (the “Authority”) is seeking to procure a Contract for Assertive Outreach Service for Rough Sleepers. The specification is outcomes based, and the Service Provider is expected to offer an innovative and creative solution to meet support needs, ensuring all possible outcomes are achieved as a priority whilst also seeking value for money, providing for the following packages of support. Service Providers must offer efficient savings over a long period of time, ensuring that as people become more independent and require less support, the packages of support are tapered down to reflect this. The Authority is seeking to appoint a suitable Service Provider to deliver a Housing Related Support (HRS) Assertive Outreach Service to Rough Sleepers. The Housing Support Grant provides housing-related support to help vulnerable people to live as independently as possible within their own homes, specialist housing or sheltered housing. The program delivers a pro-active approach, aiming to prevent problems in the first instance by providing help as early as possible to reduce demand from other services such as Health and Social Services.
