Description
1. Przedmiotem zamówienia jest wykonanie jednorazowych przeglądów technicznych i związanych z nimi konserwacji mających na celu utrzymanie w pełnej sprawności techniczno-eksploatacyjnej sprzętu medycznego i sprzętu niemedycznego mającego zastosowanie w medycynie znajdującego się na wyposażeniu SPSK im. Prof. A. Grucy CMKP w Otwocku 2. Zamówienie zostało podzielone na 95 pakietów: 1. Wykonawca zobowiązany jest złożyć na dzień składania ofert: Formularz ofertowy z wykorzystaniem wzoru stanowiącego Załącznik nr 1 do SWZ b) Formularz cenowy z wykorzystaniem wzoru stanowiącego Załącznik nr 1A do SWZ. c) Autoryzacja producenta sprzętu w zakresie wykonywania napraw serwisowych dla serwisu wykonawcy - dokument ma umożliwić dokonanie oceny w kryterium K 2 „Posiadanie autoryzacji producenta sprzętu w zakresie wykonywania napraw serwisowych dla serwisu wykonawcy będącego przedmiotem składanej oferty” opisanym w rozdz. XV ( o ile dotyczy). d) Oświadczenie Wykonawcy (JEDZ) - wg wzoru stanowiącego załącznik nr 2 do SWZ. e) Oświadczenie podmiotu udostępniającego zasoby (JEDZ) - wg wzoru stanowiącego załącznik nr 2 do SWZ.
