Description
Region Halland har för avsikt att upphandla och implementera en tjänst gällande ett digitalt medicintekniskt hjälpmedel i hemmet för egenvård av hjärtsvikt. Hjälpmedlet ska användas för daglig viktkontroll och för att stärka hjärtsviktspatientens egenvårdsbeteende, genom att hjälpa patienten från att vara en passiv konsument av vård, till en aktiv del i vårdkedjan.
