Description

Przedmiotem zamówienia są sukcesywne dostawy materiałów zużywalnych do mechanicznej trombektomii naczyń mózgowych wraz z utworzeniem depozytu dla Zespołu Opieki Zdrowotnej w Bolesławcu. Szczegółowy opis przedmiotu zamówienia zawiera Formularz cenowy - Załącznik nr 2 do SWZ. Parametry opisane w Załączniku nr 2 do SWZ jako wymagane parametry techniczno-użytkowe, stanowią szczegółowy opis przedmiotu zamówienia, których niespełnienie spowoduje odrzucenie oferty jako niezgodnej z warunkami zamówienia na podstawie art. 226 ust. 1 pkt 5 ustawy Pzp. Wykonawca zobowiązany jest do utworzenia depozytu w miejscu wskazanym przez Zamawiającego (Magazyn Medyczny), w terminie do 3 dni roboczych od dnia zawarcia umowy. Przy pierwszej dostawie należy uwzględnić dostawę depozytu- we wszystkich rozmiarach wskazanych przez Zamawiającego. Depozyt będzie sukcesywnie uzupełniany po każdym zabiegu o wykorzystany zestaw lub jego poszczególne elementy. Zamawiający nie dopuszcza możliwość składania ofert częściowych. Każdy Wykonawca może złożyć tylko jedną ofertę na całość zamówienia w formie pisemnej pod rygorem nieważności. W przypadku złożenia przez Wykonawcę więcej niż jednej oferty, wszystkie oferty tego…
