1. Przedmiotem zamówienia jest : zakup o charakterze sukcesywnym sprzętu medycznego jednorazowego i wielokrotnego użytku –wraz z jego dostarczeniem na koszt i ryzyko Wykonawcy do Magazynu SPZOZ w Sanoku, ul. 800-lecia 26, z podziałem zamawianego asortymentu na poszczególne pakiety, Wspólny słownik zamówień: CPV – 33.19.00.00-8 - różne urządzenia i produkty medyczne Zaoferowane wyroby muszą być dopuszczone do obrotu i stosowania zgodnie z obowiązującymi przepisami. 2. Szczegółowy opis, zakres i wielkość przedmiotu zamówienia oraz warunki realizacji zamówienia zostały zawarte w formularzu przedmiotu zamówienia, cenowym stanowiącym załącznik nr 2 oraz wzorze umowy stanowiącym integralną część SWZ. 3. Podane w formularzu cenowym ilości są wartościami szacunkowymi/prognozowanymi, które w trakcie obowiązywania umowy mogą ulec zmniejszeniu w zależności od aktualnych potrzeb Zamawiającego. Zamawiający w okresie obowiązywania umowy gwarantuje zamówienia na poziomie minimum 25% wartości zawartej umowy. 4.
