Description

Przedmiot zamówienia stanowi dostawa wyrobów medycznych (śrubki, płytki stomatologiczne). Szczegółowy opis przedmiotu zamówienia, opis wymagań zamawiającego w zakresie realizacji i odbioru określają: - opis przedmiotu zamówienia - załączniki asortymentowo – cenowy – załącznik nr 1-3 do SWZ - projektowane postanowienia umowy – załącznik nr 5,6 do SWZ Dla Pakietu 1 oraz 2 Wykonawca we wskazanym przez Zamawiającego terminie, na czas trwania umowy tj. 24 miesięcy, bezpłatnie dostarczy do siedziby Zamawiającego i bezpłatnie użyczy niezbędne instrumentarium wymagane do wszczepienia elementów zespalających i rekonstrukcyjnych do zabiegów w obrębie twarzoczaszki (projekt umowy użyczenia stanowi załącznik nr 6 do SWZ).
