Description
1. Przedmiotem zamówienia jest dostawa sprzętu medycznego jednorazowego użytku z możliwością składania ofert częściowych – zgodnie z wyszczególnieniem w formularzu asortymentowo-cenowym, stanowiącym Załącznik nr 3 do SWZ (pakiety nr 1-34). 2. Przedmiotem zamówienia jest także: dzierżawa aparatu do biopsji mammotomicznej pod kontrolą usg (pakiet nr 16) wraz z obsługą techniczną i serwisową. Koszt obsługi ponosi Wykonawca. Umowa dzierżawy stanowi załącznik nr 4a do SWZ. 3. Zaoferowane wyroby medyczne, muszą być dopuszczone do obrotu i używania na terenie Polski zgodnie z obowiązującymi przepisami prawa i posiadać dokumenty potwierdzające przeprowadzenie odpowiedniej procedury oceny zgodności z wymogami określonymi w ustawie z dnia 7 kwietnia 2022 r. o wyrobach medycznych (t.j. Dz. U. z 2024 poz. 1620) i być oznaczone znakiem CE, a w przypadku asortymentu do ochrony osobistej dodatkowo spełniał wymagania bezpieczeństwa i ochrony zdrowia zawarte w Rozporządzeniu (UE) 2016/425, dla środków ochrony indywidualnej, a wszystkie użyte materiały posiadały niezbędne atesty i dopuszczenia do stosowania w jednostkach służby zdrowia.
