Description
Regulated Community Care and discharge Capacity (adults). Delivery of individualised and personalised care planning, individual support in the community/at home, discharge capacity. This provider was considered against the 5 key criteria above. These criteria are not weighted as they are equally important for this service. They clearly meet and in some elements exceed the requirements of this service. No change to the contract to any significant level and will allow greater system support and therefore VFM.
