Description
The City of Bradford Metropolitan District Council requires delivery of a Multiple Needs Navigator Service which will provide intensive, person-centred support to Individuals experiencing several overlapping and persistent challenges and which necessitate an integrated, co-ordinated support approach, across multiple services. Individuals accessing the Service will typically be experiencing 3 of the following: homelessness or being at risk of; problematic alcohol and/or drug use; mental ill-health; experience of domestic abuse or trauma; involvement in the criminal justice system. The Service will contribute to a wider system change, by identifying barriers, influencing practice and working in partnership with local Services and organisations to ensure Services are coordinated, accessible and trauma informed.
