Description
Hospitals continue to see, in line with the national picture a surge in demand leading to hospital corridors being transformed into temporary bed space with a knock-on effect to supporting appropriate but speedy discharge of patients who were medically fit. In a response to the system wide pressure Cheshire East Council directed its Community Connectors to work collaboratively with the two NHS trusts at Leighton and Macclesfield Hospital and its Adult Social Care Team to reduce the impact on these two areas. This service specification covers all service users of the Cheshire East Council Community Support Connectors Hospital Discharge and Adult Social Care workstreams. All service users will be adults (18+), be Cheshire East residents who are patients at Leighton or Macclesfield Hospital, or for Adult Social Care Referrals, fall in the scope of Cheshire East Councils Adult Social Care team.
