Description
On behalf of the Renfrewshire Health & Social Care Partnership (RHSCP), NHSGGC is looking for a provider to offer a Community Link Worker Service in collaboration with all General Practices in Renfrewshire. Renfrewshire HSCP aims to commission a Community Link Workers Service in Renfrewshire to support strategic planning and enhance health outcomes.
