Description
Cardiff Council is seeking to understand the market from providers who are committed to promoting services that meet the needs of, and promote and maintain the independence and inclusion of, older people and vulnerable adults who may have dementia, other forms of cognitive impairment, physical and sensory impairment and or mobility issues, whose needs will range from targeted individual support to complex needs. Extracare and Supported Living recognises that all older people and vulnerable adults, regardless of the level or type of care need, are able to make choices about how to live their lives. These services enable individuals to live as independently as possible. Care and support is personalised and outcome focused. The vision is for a locality approach based on the benefits of care services working closely with preventative services, community health and social work teams, community hubs and primary care clusters to achieve the best preventative and care outcomes for people.
