Description
Bournemouth, Christchurch and Poole Council (BCP Council) is seeking providers to join the new Outcomes-Focused Framework for Care and Support at Home, designed to transform the way care is delivered across our communities. This updated approach places people, their strengths, and aspirations at the heart of everything we do. BCP Council Adult Social Care's (ASC) vision is to support people to live fulfilling lives, in ways they choose, and in environments where they feel safe and connected. The Care and Support at Home Framework is committed to a progressive approach that will embed outcomes focus throughout the duration of this contract. This means moving away from the traditional 'time and task' model of care towards a model that is person-centred, flexible, and empowering. The Framework is designed to provide comprehensive support for all adults aged 18 years and over within the BCP Council conurbation, whose needs can be met through Care and Support at Home services, and do not require highly specialist or complex interventions.
