Description
The Home from Hospital service is a highly collaborative and coordination-focused service providing, flexible practical support (not personal care) for a period of up to 4 weeks. The service is for vulnerable Wiltshire residents (aged 18+) who are medically fit for discharge but unable to return home due to practical barriers. The service runs from Monday to Friday with flexibility to support over weekends when there are Hospital pressures. The service is responsive and works effectively with a range of partners to facilitate discharges. The successful service provider will have skills in navigating and coordinating solutions that prevent an individual being discharged home. Tasks are likely to include: • Presence within each of the hospitals in Wiltshire (GWH, RUH and SFT) and the community hospitals • Meeting with individuals on wards to understand barriers to their return home and being innovative in co-producing resolutions • Supporting individuals to pay for services or goods to support their discharge home (food, locksmiths, deep clean services) • Signposting to advice and support around benefits and accessing emergency funding • Linking up with the individual with comm…
