Description
This notice is an intention to award notice under the Provider Selection Regime competitive process. The purpose of this Service is to deliver care within a Residential Care Home with Nursing (referred to from this point as a Nursing Home) for those patients who have been assessed as eligible for NHS Fast Track funded care, have expressed a preference for care in a Nursing Home and whose needs can be safely and appropriately managed in a Nursing Home. The Service is commissioned to provide dignity and assurance to people and their families when they are entering their final weeks of life and is expected to be safe, responsive and efficient. This includes enabling patients to achieve the best possible quality of life whatever their disease or disability, gender, ethnic background, or sexual orientation. This will be achieved through the application of a holistic person-centred approach to care which is integrated seamlessly with all other health services, especially GP primary medical services and specialist palliative care services.
