1. Przedmiotem zamówienia jest przygotowanie i przeprowadzenie części terenowej „Badania zagrożenia populacji polskiej chorobami układu krążenia – WOBASZ” wraz z transportem materiałów do Centralnego Laboratorium Narodowego Instytutu Kardiologii Stefana kardynała Wyszyńskiego - Państwowego Instytutu Badawczego (NIKard) w Warszawie. Przedmiot zamówienia, o którym mowa w zdaniu pierwszym, został podzielony na 5 następujących po sobie etapów.
