1. Przedmiotem zamówienia jest zakup i sukcesywna dostawa leków z programów lekowych oraz chemioterapii wyszczególnionych w częściach dla Uniwersyteckiego Szpitala Klinicznego im. Fryderyka Chopina w Rzeszowie, a w tym: Część nr 1 - Omalizumabum 75 mg /0,5 ml; 150 mg /1 ml INJ amp-strz. Część nr 2 - Polatuzumabum vedotinum i.v. 140 mg; 30 mg fiol. Część nr 3 - Trastuzumabum emtansinum i.v. 100 mg; 160 mg fiol. Część nr 4 -Panitumumabum i.v. 400mg/20 ml; 100mg/5 ml fiol. Część nr 5 - Daratumumabum roztwór do wstrzykiwań 1800 mg (120 mg/ml) fiol. 15 ml Część nr 6 - Mosunetuzumabum i.v. 1 mg/1ml; 30 mg/30ml fiol. Część nr 7 - Ivosidenibum 250 mg tabl.
