Description
De gemeente Meppel is verantwoordelijk voor de inkoop van jeugdhulp op grond van de Jeugdwet. Binnen deze verantwoordelijkheid staat het belang van het kind en de continuïteit van zorg voorop. Voor de cliënt 1 is sinds 17 mei 2024 en voor cliënt 2 is sinds 16 mei 2025 jeugdhulp ingezet via Rosales Zorg. Deze hulp bestaat uit het aanbieden van een woning met 1 op 1 begeleiding waar cliënten alleen verblijven. Cliënten hebben dus geen groepsgenoten. Naast deze 1 op 1 begeleiding is er een GZ psycholoog betrokken die wordt ingezet voor zowel ondersteuning begeleiding als gesprekken met cliënten en ouders. De begeleiding helpt cliënten bij het stabiliseren en oppakken van hun leven. Er is grote onzekerheid hoe lang deze zorg nodig is. Doelstelling is om de situatie eerst te stabiliseren en vervolgens geleidelijk af te schalen. Echter is op voorhand niet te zeggen of en wanneer dit kan. De looptijd is maximaal tot de cliënten de leeftijd van 18 jaar bereiken. De zorg is voor 2 cliënten (individueel geplaatst).
