Wmo begeleiding is gericht op de begeleiding van personen vanaf 18 jaar met een beperking of met (chronische) psychische of psychosociale problemen. De begeleiding vindt zoveel mogelijk plaats in de eigen leefomgeving van de cliënt. Voorliggende voorzieningen en ondersteuning in de sociale basis zijn voor deze cliënt niet voldoende passend. -
