Description
3. Opis przedmiotu zamówienia 1. Przedmiotem zamówienia jest dostawa preparatów do żywienia dojelitowego i pozajelitowego wraz z osprzętem oraz mleka modyfikowane zgodnie z podziałem na 65 zadań. Szczegółowy opis przedmiotu zamówienia stanowi: Załącznik nr 1 do SWZ– „Formularz asortymentowo-cenowy” Szczegółowe warunki i zasady realizacji przedmiotu zamówienia określają dodatkowo „Warunki umowne”, będące Załącznikiem nr 5 do SWZ. 2.Zamawiający WYMAGA DODATKOWO, na okres obowiązywania umowy, dzierżawy dla: 1) Zadania nr 1 – 30 pomp, do stosowania enteralnego zarówno przy łóżku pacjenta jak i przenośnego. 2) Zadanie nr 4 – 60 pomp przeznaczonych do żywienia enteralnego w warunkach szpitalnych lub domowych. 3) Zadanie nr 6 – 25 pomp przeznaczonych do żywienia pozajelitowego w warunkach szpitalnych lub domowych. 4) Wymagania jakie muszą spełnić pompy zamieszczone są w Załączniku nr 1 do SWZ. 3. Szczegółowe warunki i zasady realizacji przedmiotu zamówienia określają dodatkowo Warunki Umowne będące załącznikiem nr 5 do SWZ „Warunki umowne”. UWAGA !!!!!!
