Description
Przedmiotem zamówienia jest dostawa wyrobów medycznych jednorazowego użytku dla potrzeb Centrum Pulmonologii i Torakochirurgii w Bystrej, szczegółowy opis przedmiotu zamówienia został opisany w asortymencie i ilościach określonych szczegółowo w formularzu asortymentowo-cenowym stanowiącym załącznik nr 2 do Specyfikacji Warunków Zamówienia (SWZ), a także w rozdz. II ust. 1 SWZ. Przedmiot zamówienia został podzielony na 73 pakiety. Zamawiający dopuszcza składanie ofert częściowych. Każdemu z wykonawców przysługuje możliwość złożenia oferty na dowolną ilość wybranych przez siebie Pakietów, z zastrzeżeniem, że oferta musi zawierać wycenę całego asortymentu w ramach Pakietów, na które Wykonawca składa ofertę. Zamawiający wymaga aby cały asortyment był nowy i nieużywany. Wszystkie oferowane materiały medyczne będące wyrobami medycznymi muszą być wprowadzone do obrotu i stosowania na terytorium RP, sklasyfikowane według obowiązujących przepisów.
