Description
1. Przedmiotem zamówienia jest dostawa wyrobów medycznych do radiologii zabiegowej, kardiochirurgii, kardiologii inwazyjnej i elektroterapii. 2. Przedmiot zamówienia składa się z 80 części, których szczegółowy opis, asortyment oraz ilości zawiera załącznik nr 1 do SWZ – Formularz asortymentowo-cenowy, który Wykonawca załącza do oferty. Wykonawca wypełniając wskazane pozycje w niniejszym załączniku potwierdza spełnianie wymagań określonych przez Zamawiającego. 3. Sposób realizacji przedmiotu zamówienia określa projekt umowy- załączniki nr 3 do SWZ, mianowicie: 3A – sprzedaż - dot. części nr 1 , 3, 6, 12, 13, 14,15, 16, 18, 21, 25-28, 32, 34, 35, 37, 40-43, 46, 48, 49, 53-61, 71-77, 79, 80, 3B – sprzedaż z użyczeniem - dot. części nr 23, 44, 3C – komis bez użyczenia - dot. części nr 2, 5, 7-9, 11,17, 19, 20, 22, 24, 29-31, 33, 38, 39, 47, 50, 51, 62-70, 3D – komis z użyczeniem - dot. części nr 4, 36, 52, 3E – komis bez użyczenia z dzierżawą- dot. części nr 10, 3F – sprzedaż z dzierżawą - dot. części nr 45, 78. 4.
