Description
Przedmiot zamówienia stanowi zakup leków, środków o charakterze dietetycznym przeznaczenia medycznego, wyrobów medycznych oraz surowców farmaceutycznych do apteki szpitalnej SWPZZPOZ w Radomiu. 1) Zamawiający wymaga złożenia oferty uwzględniającej zapisy Ustawy z dnia 12 maja 2011 r. o refundacji leków, środków spożywczych specjalnego przeznaczenia żywieniowego oraz wyrobów medycznych (Dz. U. z 2022 r. poz 357 ze zm.) 2) Zakres przedmiotu zamówienia obejmuje: Załącznik Nr 1 – Zadania od 1 do 21 (Formularz cenowy) 3) Szczegółowy opis przedmiotu zamówienia, opis wymagań zamawiającego w zakresie realizacji i odbioru określają: 4) Załącznik Nr 1 Zadania od 1 do 21 - (Formularz cenowy) 5) Projektowane postanowienia umowy – załącznik nr 4 do SWZ. 6) Przedmiot dostawy musi być dopuszczony do obrotu i używania.
