Description

De overeenkomst bestaat uit het leveren van ambulante jeugdhulp welke is onderverdeeld in de volgende categorieën: Ambulante (systemische) hulpverlening; Groepsproducten; Woonproducten; Overige producten. Binnen iedere categorie zijn verschillende producten benoemd. De productbeschrijvingen ambulante jeugdhulp en wonen Midden Limburg 2026 e.v. zijn als bijlage 2 opgenomen bij dit inkoopdocument. Dit document bevat naast de beschrijving van de producten ook eisen die gesteld worden aan de uitvoering en degene die de hulp uitvoert. Tevens wordt ingegaan op doel en inzet van de ambulante hulpverlening en de algemene uitgangspunten voor de tarieven. Daarnaast is als bijlage 8 het Werkafsprakenboek ambulante jeugdhulp en wonen Midden-Limburg opgenomen.
